Congregational Care Network

Congregational Care Network (CCN), a program offered through IU Health, is designed to bridge that gap by providing consistent emotional support to those in need.

MISSION:

Congregational Care Network (CCN) provides support to IU Health patients who are lonely or in need of additional help after being discharged from the hospital. Volunteers, known as connectors, are trained to:

Provide compassionate, listening support.

Help companions ease back into their at-home routines.

Identify sources of support within companions’ existing networks.

Connect companions to new resources and community opportunities.
